A community health assessment (CHA) is a participatory process that brings together tribal departments, external partners and community members to develop a shared understanding of community health. It uses a systematic process to collect data and provide a sound basis for decision making and action to improve health. When aligned with an Indigenous worldview, it can help tribes understand the factors and root causes that contribute to health risks, build on community strengths, and inform community health improvement plans strategies.
